To complete the survey online, please follow these simple steps:
Step 1: Get What You Need to Start
If your child attends an NYCPS or Charter School, you will need your child’s nine-digit student identification number (OSIS) to begin the survey online. You can find the OSIS number on your child’s report card. If you cannot locate it, please reply to this email at Lcardenas3@schools.nyc.gov with your child’s name and class.

Step 2: Enter Your Access CodeNYCPS and Charter School Families: https://surveys.panoramaed.com/nycdoe/login
Enter a lowercase “f” followed by your child’s nine-digit OSIS number (example: f123456789).

Step 3: Complete the Survey

Complete the survey using the instructions at the top of the form. Return your completed survey using the prepaid envelope included with the survey materials. You may send it back to school with your child or mail it directly to:
